West Berkshire Everyone Active Discount

You can apply online or pick up an application form from any Everyone Active centre run in partnership with West Berkshire Council and complete and sign your form. If you’re under 16 years old, a parent or guardian must sign your application. Then, take your completed form to one of our leisure centres, along with the following supporting documents:

• Proof of address – a driving licence, bank statement, building society book, family allowance, rent book, council tax book, tenancy agreement, gas/electric/water/telephone bill, credit card bill, TV licence, or a medical card – please note that bills and statements must be dated with the last three months)
• Proof of identity
• Proof of age (for junior or senior cards)

If you’re applying for the concessionary West Berkshire Everyone Active discount, you must also bring proof that relates to your particular circumstances:

• If unemployed or retired, a bus pass and/or proof of retirement/unemployment
• If disabled, a disabled person’s bus pass or doctor’s letter
• If you’re on a low income, you’ll need proof in the form of a receipt for income support, housing benefit, invalidity benefit, job seekers allowance, or council tax benefit
